Further enhancing our team approach, CCP integrated behavioral health provides integrated behavioral health care following the evidence-based primary care behavioral health model (PCBH). The program is under the direction of Dr. Cynthia Stone, DBH, LMHC, NCC.
Integrated behavioral health following the PCBH model is a fully integrated model of health care delivery that engages people in the full range of physical, behavioral, preventive, and therapeutic services to produce the best outcomes, while also proving to be the most effective approach to caring for the whole person.
In our integrated primary care practices, behavioral and medical providers work together using a systematic and evidence-based approach to provide patient-centered care. This care may address mental health issues, substance abuse conditions, health behaviors (including their contribution to chronic medical illnesses), life stressors, crisis, stress-related physical symptoms, and other behavioral issues. Physical/Behavioral Integration is proven as a good health policy and good for health, providing our patients optimal care.
