Community Care Physicians offers access to licensed mental health counselors, board-certified psychiatrists, and clinical social workers as Behavioral Health Consultants (BHCs) within most of our practices. These Behavioral Health Providers assist our patients with mental health issues, stress, coping, behavior and lifestyle change, and the behavioral management of chronic disease.
We offer behavioral health services using telemedicine or video visits using your smartphone, tablet, or computer. It’s secure and doesn’t require any app or software downloads.
How Do I Get Connected with a Behavioral Health Consultant?
Many patients are referred by their doctors.
You can ask your doctor for a referral, or your doctor might recommend seeing one of our behavioral health consultants and your doctor will place a referral. With your doctor’s referral, you will be contacted to get connected with one of our behavioral health consultants.
Patients can also self-refer.

Within a short time, you will be connected to one of our licensed counselors.
Counseling Services at Wellness Way in Latham, NY
Integrated Behavioral Health has opened a practice at Wellness Way in Latham, NY, to further support our Community Care patients. Wellness Way offers:
- Traditional longer-term counseling
- Specialty counseling to include, but not limited to:
- Couples counseling
- Substance abuse counseling
- Counseling for patients with chronic medical issues
Our team will determine the best place to see you – in your CCP practice, at Wellness Way, or via telemedicine virtual visit.
